Introduction: Defect of hospital information systems leads to decrease income in public hospitals, such that more than 30% of the services provided are not reimbursement. The purpose of this article was to identify hospital resources wastages in health transformation plan (HTP) in Iran, and how to avoid the waste of resources and cost-containment approaches in the field of health information and medical record.Methods: The present original research was a kind of content analysis and applied studies. In order to identify hospital resources wastage and cost-control approaches, PubMed, Irandoc, Scientific Information Database (SID), Google Scholar, and Magiran databases were searched using the keywords of Information, Records, Cost and Cost Control, health transformation plan, and related articles were extracted. Nominal group technique was used to confirm and categorize wastage, and a focused group discussion was used to confirm the approaches.Results: The most considerable resource wastage factors in the field of medical information and records were duplicate and inconsistent administrative requirements, dispersion of patient information, and increase the insurance deductions; and the most important cost-containment approaches included mapping the right route of data from hospital to Ministry of Health, creating standard software for validation and claim status, setting up a notification system with patient-centered patient programs, and reducing the insurance deductions.Conclusion: Medical records and hospital information are the basis of all evidence-based decisions. Since today's financial management is much more important than before, also due to resource constraints, it seems that it is possible through reducing costs by eliminating or reducing waste of resources.

Consuming medical services is labeled as utilization. Utilization studies are used by insurance companies to study the behavior of medical service consumers. Currently, insurance companies focus more on caregivers' behavior, and neglect the consumers’ side. This was a narrative review study carried out in year 2018 to emphasize the necessity of utilization reviews and their importance. In this study, we used ProQuest, ScienceDirect, PubMed, Scopus, and ISI Web of Science databases without any time limitation. We chose 50 articles and reviewed them to found 5 essential questions about utilization reviews, and their importance, differences, and challenges, as well as factors affecting them. Our study revealed that having standardized data formats for service providers, integrated data pools, and relevant software sensitive to unusual service consumers, facilitate the utilization review studies.

Introduction: One of the 8 service packages in the first phase of Iran health transformation plan (HTP) is dedicated to improving the quality of outpatient visits. We investigated the average visits/hour criterion at the Alzahra university hospital, Isfahan, Iran, as the biggest province health center, after implementation of health transformation plan.Methods: This descriptive cross-sectional study, conducted in all 26 clinics of Alzahra hospital using visit/hour as the main indicator for evaluating the quality of outpatient services. Data gathered from June 2014 to July 2015 from clinic secretaries with the help of a checklist. Gathered data were analyzed via calculating the quarterly mean and standard deviation of visits/hour separately for internal and surgical clinics using SPSS18 software.Results: Considering 8 visits/hour as the desired standard number, the mean visits/hour in the internal clinics in the first, second, third and fourth quarter were 6.06 ± 2.27, 7.76 ± 4.80, 7.02 ± 4.18 and 6.37 ± 2.52, respectively. For the Surgical clinics, the mean visit/hour was 9.88 ± 6.09, 10.11 ± 5.47, 9.40 ± 6.26 and 7.63 ± 2.93 for first, second, third and fourth quarters, respectively. Conclusion: After implementation of health transformation plan in Alzahra hospital in May 2014, the visits/hour criterion worsened during the second quarter but improved in the third and fourth quarters. This can be due to the increase in the number of patients at the start of the plan or financial issues and the effects of which was reduced due to better supervision later on. We suggest using different standards for surgical and internal clinics in order to improve the effectiveness of the plan.Keywords: Hospitals; Visit; Outpatients; Health Transformation Plan; Iran

Introduction: Out of pocket payments (OOP) as a kind of financial resources in health systems has regressive effects on health status, equity and both qualitative and quantitative health system performance indicators. Higher health expenditures and higher OOP may lead to more people fall in poverty or decrease their accessibility to health services. Unfortunately this could be found more among poor people whom need more protection. This study aimed to reveal if social security organization (SSO) wants to protect their insured people from higher amounts of OOP (bills more than 100 million Rials) how it would affect cost coverage of other people assuming fixed total cost of all bills for SSO. Methods: That was a cross sectional study on Isfahan hospital bills for inpatients of SSO during 2008. Sampling method was cluster sampling. We studied 2430 hospital bills and copayment paid directly by patient. The data were collected by Finance information form. The form was a standard so it is the reliable and valid. Data analyzed with Excel software and descriptive statistics. Results: Now SSO covers around 90% of hospital bills for its insured patients regardless of bill amount. Our calculations according to before mentioned show that SSO should pay less to low cost bills through one of these scenarios: First: for all bills fewer than 10 million Rials, covering 43% of costs. Second: for all bills fewer than 20 million Rials, covering 77% of costs. Third: for all bills fewer than 100 million Rials, covering 86% of costs. Conclusion: Findings show that social security organization by using Variable Copayment in different manners could protect individuals with high health expenditures and protect households against catastrophic costs.Key words: Hospitals; Social Security; Costs

Introduction: Hospitals are required to focus on core activities which are inevitable affair. The sensitivityof the monitoring and supervision mechanism as well as preserving and enhancing the quality ofoutsourced services prevent new problems in this area.Methods: This was a descriptive and applied study which was done by case study (pretest-posttest)method. Outsourcing nursing services of clinical wards (ENT and surgery) of Kashani Hospital wasstudied. Data were extracted from the questionnaire (internal customer's satisfaction), semi-structuredinterviews with officials and also survey of financial and administrative documents as well as recordsrelated to the topic. The validity of thee questionnaire was approved using the standard form of thesatisfaction in Isfahan University of Medical Sciences, Iran and its reliability was based on calculatingCronbach’s alpha.Results: The findings indicated an increased number of graduate nurses per bed to fulfill the main goal ofoutsourcing. However, achieving this goal was accompanied with increasing costs per bed and resulted inincreased total costs of the hospital after outsourcing. In addition, the intangible changes in internalcustomer satisfaction with the performance of non-governmental sector have been achieved.Conclusion: While outsourcing should bring about staff and patients' satisfaction and increase theefficiency and effectiveness, outsourcing nursing workforce strategy led to severe loss of efficiency.Therefore, the adopted strategy did not meet the productivity for the hospital

Introduction: Studies of design location are done in order to respond better to the needs of the physicalspaces of the hospital. The present study aimed to improve workflow processes of emergency departmentthrough the study of designing the location in restructuring of the emergency department of Shariati Hospitalin 2011 as one of the largest hospitals in Isfahan Province, Iran that has to deal with emergency patients.Methods: This was a descriptive case study related to Emergency Department of Shariati Hospital in2011. Data were collected in 4 Parts, i.e. reviewing the duties and functions, workflows, challenges andexisting spaces and design changes. According to the clinical and supportive areas in emergency plans in2011, data were collected through standardized questionnaire and direct observations to codificationprocesses. Data were analyzed using Excel Software.Results: Out of six objectives of restructuring, setting up triage had the first priority and among processesand spaces, emergency patient admission process, toxicity admission process, children admission process,entrance, triage, critical and noncritical patient section, had the most fundamental changes, respectively.Conclusion: According to the accreditation standards, it is essential for hospital managers to payparticular attention to the building standards, which is one of the major causes of patient satisfaction. Theymay use the simple, comprehensive and applicable method of design location plans in hospitalrestructuring, process improvement, and develop building quality improvement program according toaccreditation standards.

Introduction: Human skills, knowledge and efficiency are important factors in the success oforganizations and community health centers which have a critical role in total qualitymanagement (TQM) as a management system. Knowledge is necessary as a source for theorganizations survival. On the other hand, in order for implementation of knowledgemanagement, there is a need for a relatively long period, providing intellectual and culturalcontexts, educational skills and knowledge and technological centers. This study aimed todetermine the relationship between TQM and knowledge management from the perspective offaculty members of Isfahan University of Medical Sciences, Iran.Methods: This was a descriptive-correlational study. The study subjects included the total offaculty members of University of Isfahan (491 people) and Isfahan University of MedicalSciences (594) during academic years 2009-2010 who were selected through proportionatestratified random sampling (300). Data collection tools included a Baldrige questionnaire basedon the TQM model and knowledge management components based on Pastor standardquestionnaire. The content and face validity of questionnaires were confirmed and reliabilitythrough Cronbach's alpha coefficient were obtained as 0.87 and 0.91, respectively.Results: Mean of all the TQM criteria were higher than average in University of Isfahan exceptleadership, strategic planning and human resource management. It was the same in IsfahanUniversity of Medical Sciences except criteria like focusing on customer, information analysis,strategic planning and process management. All the components of knowledge management weremore than average level in Isfahan University of Medical Sciences, and except refining theknowledge in University of Isfahan. Correlation coefficient between scores of TQM and knowledgemanagement components was significant (P ≤ 0.05). In addition, multiple regression showed thatthe correlation between TQM and knowledge management was statistically significant.Conclusion: TQM criteria can be effective in line with appropriate functioning of knowledgemanagement and its impact on organizational performance can also increase the efficiency of theorganization

Introduction: Policy makers and decision makers of medical tourism must identify stakeholders who canbe affected by and can affect this industry and determine their status for partnership. The aim of this studywas to identify and analyze the main stakeholders of medical tourism in Isfahan, and provide strategies fordeveloping this industry.Methods: A qualitative study was conducted in 2011. At first, key stakeholders of medical tourism inIsfahan were identified according to experts idea and literature review. Then we interviewed these keystakeholders. Data analysis was conducted using stakeholder analysis and development strategies wereprovided for medical tourism.Results: The key stakeholders of medical tourism in Isfahan were divided into 9 categories. Theyconsisted of Governor of Isfahan Province, Cultural Heritage, Handcraft and Tourism Organization ofIsfahan, Chamber of Commerce, State Medical Council, Isfahan University of Medical Sciences, healthservices providers, tourism services providers, private investors, and the Export Development Bank ofIran. Stakeholders’ characteristics showed that Cultural Heritage, Handcraft and Tourism Organization ofIsfahan, clinic, and International Relations Office of the Isfahan University of Medical Sciences had fullknowledge of the government policy about medical tourism. Private hospital, State Medical Council,investors, and University of Medical Sciences had great power. Private hospital, clinic, Cultural Heritage,Handcraft and Tourism Organization of Isfahan, and University of Medical Sciences had a supportiveposition. Effective strategies included strategies focused on increasing power, increasing support, andmaintaining their position.Conclusion: There are different stakeholders in the medical tourism industry of Isfahan. Therefore, policymakers can plan, make policies and decisions, and use effective strategies in order to develop medicaltourism by designing a medical tourism stakeholders’ network and a medical tourism provincial council,and by clarifying the roles and responsibilities of stakeholders.

Introduction: Management is the most important factor in success and survival of organizations.Managers thus have decisive vital role in development of any organization. The purpose of this research was to assess the opinions and perceptions of executives and academic experts about Islamic management components.Methods: This study used qualitative content analysis to evaluate 14 academics who had experience or publications related to the topics of the research. To collect data, interviews were conducted to reach saturation. The main concepts of the interviews were then extracted.Results: The religious, Islamic government aims to restore peace, security, and social rights, not to intimidate people. The leader of an Islamic system should thus be committed to human dignity, values, regulations and divine laws. Islamic management appreciates evaluation, unity and harmony, tolerance, and consultation and communication with people. Mercy, compassion and kindness to the citizens are the prerequisites of an Islamic government.Conclusion: Islamic management is derived from the guidelines of Islam. The most important feature of this management method is to consider a holy goal for which no unacceptable measures should be performed.

Introduction: Medical tourism is a complex subject for studying. The aim of this study was to do systematically review, describe, and analyze medical tourism articles from all over the world. Methods: In order to perform this systematic review, the databases of Institute for Scientific Information (ISI), Science Direct, Emerald, Oxford, Magiran, and Scientific Information Database (SID) websites were searched in 2000-2011. The retrieved articles were collected and classified using descriptive statistics and content analysis. Results: Among the total 28 studied articles, 11 original articles, 3 case studies in Mexico, India, Hungary, Germany, and Iran, and 14 narrative reviews were found. The articles mainly focused on the definition of medical tourism, medical tourists' motivations and development factors, ethical issues in medical tourism, the impact of medical tourism on health system, and medical tourism marketing. Conclusion: Our findings indicated similar definition of medical tourism in all articles despite various motivations of medical tourists. Nevertheless, there are a wide range of interests and consequences in studies, as more studies of developing countries showed the benefits of medical tourism while more developed countries pointed out the consequences of medical tourism. Ultimately, the subject of medical tourism varied according to policies and programs in each country. Therefore, comprehensive and detailed research is required to clarify the issue within the country.

Introduction: The process of knowledge transfer and exchange (KTE) in fact is a process of utilizing knowledge gained from research to practice. Appropriate exploitation of research results is guaranteed by good management of the research. The research aimed to evaluate the situation of KTE in current research centers in Isfahan University of Medical Sciences, and review its strengths and weakness in this field.Methods: This descriptive– analytical and cross-sectional study was carried out in 2010. First a KTE briefing workshop was held for officials and experts from research centers and deputy of school, about how to complete the questionnaire. The self questionnaire, prepared earlier in Tehran University of Medical Sciences, was completed by research centers. Findings were analyzed using Excel software.Results: The results were presented in four sections of a research cycle i.e.: the question of research (research priorities), knowledge production, knowledge dissemination and evidence based practice. Knowledge production by acquiring 63% of total score had the highest score and research question or setting of research priorities and having close relationship with stakeholder's with 30% of the total scores had the lowest score.Conclusion: Research centers need to obtain more efficient relationship with their stakeholders to identify and prioritize research topics. Doing granted research (particularly from sources outside the university) makes closer contact with the stakeholders and the use of research achievements is somehow guaranteed. Therefore KTE can promote transfer of research outcomes to every audience properly.Keywords: Research; Evaluation; Information Transfer; Knowledge; Universities.

Introduction: Hospitals, like most executive agencies have scarified the importance of policy, planning and supervision with their executive actions. With their errors, they waste the resources and remain away from development and competitive arena.So during recent years the Ministry of Health and Medical Education as well as other areas, in order to make rational government size, strengthening accountability of the system and efficiency of the health services, has considered the strategy of supply sources from outside of the Organization on its agenda.. Therefore in this study outsourcing of medical records services unit was evaluated from different angles.Methods: In this descriptive applied before-after study, the medical records unit of Kashani Educational Center was evaluated. Data collected by a questionnaire (internal customers), a check lists (Process Evaluation Unit, medical records defects, fulfill the requirements of 88 bylaw), semi- structured interviews with officials and by checking of financial and administrative documents and records related to the topic.Results: The findings indicated decreased medical cost per case and files defects in 4.5% and 37% of cases and improvement in unit processes in 37.4% of cases, after outsourcing the medical record unit. Other results of outsourcing medical record unit were 59% increase in customer satisfaction and 70% fulfillment of the requirements of 88 by laws.Conclusion: Keep patients information is one of fundamental principles of patient privacy rights, and combined private sector presence and the above mentioned fundamental is important point in outsourcing in this section. Success in promoting private sector efficiency, reduce unit costs and increase customer satisfaction and improve service quality indicates that if the legal principles implemented, outsourcing of this unit could be considered efficient and effectiveKeywords: Outsourcing; Evaluation; Medical Records.

Introduction: Compulsive Internet Use Scale (CIUS) is a newest and prestigious questionnaire in the diagnosis of internet addiction .The goals of this study was to investigate the reliability and validity (psychometric properties) of Persian version of CIUS in internet users of Isfahan's universities.Methods: In this descriptive and Cross- sectional research 400 Isfahan university students were enrolled by quota sampling method. Studied population completed demographic questionnaire, Persian version ofCIUS, Internet Addiction Test (IAT) and Young Diagnostic Questionnaire (YDQ).Finally, to determine the cut off point, all studied subjects were interviewed clinically based on diagnostic criteria for Internet addiction (DSM- IV- TR). After collecting and scoring the questionnaires, the psychometric features of the questionnaire analyzed using factor analysis, discriminative validity, internal consistency (Cronbach’s alpha), split half, and ROC Curve by SPSS15 Software.Results: Factor analysis revealed three factors for CIUS. It has good internal consistency (Cronbach’s alpha = 0.89), split half (r = 0.89), discriminative validity (r = 0.68) and concurrent validity (0.78,0.81).The best cut off point for this questionnaire (CIUS) was 37.Conclusion: CIUS has acceptable psychometric properties and this is a valid and reliable instrument that may be used in research on Internet addiction.Keywords: Validity; Reliability; Factor Analysis; Internet; University.

Many researchers in the universities are engaged in research and science production. Although the results of their researches are often released as articles in professional journals, long and technical reports are doubted to be comprehensible for those who need to use them. "Executive summary", as a short report derived from the final research report, is a format especially developed for knowledge users who affect or are affected by researches (research stakeholders). This short article reviewed the content and usage of executive summary in available online resources. Introduction: Utilization of information system in management is necessary. Statistical Process Control (SPC) is one of suggested tools in Quality management system. In this study, processes of medical record unit in Gharazi hospital (Isfahan) are studied using SPC tools.Methods: It is a descriptive, Retrospective and trend study which was carried out by assembling indicators for some processes in medical record unit from 2003 to 2008.After Library studies, suggested indicators by experts (for Process controlling) were collected through 2 checklists. For quantitative indicators of medical records evaluation, all admitted patients during study period and for time assessing about 1500 patients were chosen randomly. Data were analyzed using Excel software.Results: Findings showed that during study period, the average time for patient discharge order (by doctors) was at 9:54 a.m. and patients waiting time in discharge process decreased (with the average of 53 minutes). In quantitative evaluation of medical records deficits, accepted fault rate (or the base line) consumed as about 6%.Conclusion: Gharazi Hospital tries to control tolerance of its processes by using SPC tools and maintaining some internal standards as baselines. These baselines may be shared with other hospitals and help to produce national and regional standards.Keywords: Medical Records; Hospitals; Statistics; Patient Admission; Surveys; Performance Assessment.

Study of research final reports was the 2nd step, gathering special goals and tangible products. In the 3rd one, an interview was made with the researches main users. The last step was to analyze the findings and to emphasize on systematic obstacles with relevant suggestions. Results: For total of 23 researches, about 4’620’350’000 Rials paid. For 16 researches a public announcement have done, the reminders had restrict announces. Among the researchers, 17 were selected by their proposals, but 6 ones just by their Cerculem Vitaes. Only the degrees of 17 researchers were relevant to the research subjects. Conclusion: The sum of all researches done in this period, the utilization capability was “High”, but the utilization indeed was “Low”. About 38% of them were faced with some managerial obstacles and 62% had some kinds of operational ones. According to the results, choosing better subjects, documenting the main goals of the research, selecting the researchers based on their experiences and their proposals, attention to operational agenda, stability of policies, and higher administration commitment are useful suggestions for removing the obstacles. Introduction: we can facilitate hospital management and prevent organizational crises by applying quality management principles. Gharazi hospital in Isfahan set up a quality management system in 1380 and in 1381 received ISO 9001 , 2000 certificates. This study compared the quality management principles and the quality management system established in this hospital. Methods: It is a cross-sectional study which was performed in 1385. All wards of the hospital in which the quality management was carried were studied. Data were collected by a checklist and a questionnaire. Checklists were made based on quality management standards and principles and questionnaires were made based on ministry of health standards. Results: The findings indicate that what is established in Gharazi hospital based on international standards series 9000 in quality management system was consistent with the eight principles of quality management. Conclusion: Establishing quality management systems in organizations in essential for their improvement but the philosophy of their establishment must be taken into account. Introductions: Due to versatility and wide range of health care services, the social care insurance organizations can not afford undertaking all health care services for the insured. This is why additional services are taken over by private or commercial health care insurance companies. Such services are known as complementary insurance. In this study , it was decided to investigate the satisfaction of the insured using the complementary insurance and those not using it. Methods and Materials : It is a descriptive study in which the data were collected by a questionnaire. It was distributed among the insured referring to four healthcare centres : Noor superspecialized polyclinic, Shahid Shoorideh superspecialized polyclinic, Khatam-al-anbia Hospital, Mostafa Khomeini Hospital. Results: Findings of the study revealed that those insured by health care services organizations used the complementary insurance more than others. And those inpatients who used the complementary insurance had the highest satisfaction and 91% of the participants advised other patients to use the complementary insurance. Conclusion: Inpatients used the complementary insurance more frequently and suggested it to other patients. Key words: insurance – insurance coverage – patients – patient satisfaction
